Panattoni provides (premiums 100% company-paid) a basic life insurance policy for eligible employees with a benefit amount of 2-times your annual earnings, to a maximum of $400,000. If your death is a result of an accident, under the AD&D provision, there is an additional benefit equal to your basic life benefit.
It is important to keep your beneficiary information current, especially following life changes such as marital status or birth/adoption of a child. PlanSource, our online benefits tracking system, houses all information regarding your plan enrollment and coverage(s), including beneficiary information.
Voluntary supplemental life and AD&D insurance is available to you and your family at coverage levels and competitive group rates that can meet your family’s individual financial needs. Please be aware that 100% of the premiums are employee paid by you through convenient payroll deductions each pay period.
When you are first eligible to participate in this plan, you are in what is called your “initial enrollment period”. This is a significant time when you are extended the opportunity to enroll in up to $150,000 of coverage under a “guarantee issue” provision, meaning no questions or medical underwriting will be required. Please keep in mind that you will need to complete a health application and/or physical examination for approval if you do not sign up during your initial enrollment period but request this coverage at a later date.
Premiums for this coverage are based on the age of the Panattoni employee and can be found in the documents listed below.
